This privacy policy sets out how Skin Science and Health Ltd uses and protects any information that you give to us when you interact with us.

Skin Science and Health is committed to ensuring that your privacy is protected. Any information that you provide to us will be collected and used in accordance with the General Data Protection Regulation, the Date Protection Act 2018 and the Privacy and Electronic Communications and Regulations 2003.

We will not share your data with any third party except for purposes relating to the medical service we provide, for example the preparation of prescriptions at the pharmacy.

Why do we collect your personal information?

We collect information about you that helps us to deliver our medical services and treatments safely, ethically and ensure they are right for you.

When you make contact via the telephone, or e mail, the information you provide will be kept and be used to contact you regarding your enquiry,make appointment arrangements, or in regards to feedback or complaints.

Skin Science and Health Ltd will NOT share your e mail with any company outside of Skin Science and Health Ltd

We may share your information if it is in regard to fraud prevention.

What information do we collect?

By agreeing to our Privacy Policy terms and conditions, you are agreeing to your data being used by Skin Science and Health for contacting you in regards to your enquiry by e mail, or SMS or telephone. At any point, you can request to have you data removed by calling the clinic. Further details on how to do this can be found below.

If you are new to Skin Science and Health – after making contact with us, we will collect initial information, this includes:
Your email address and contact telephone number
Your skin concern
Consent and preferred method for us to contact you in regards to your enquiry
If you are already a patient of Skin Science and Health Ltd, had a consultation or have had a medical treatment before, we may collect further information from you regarding further completion/ updating of a medical questionnaire, or completing feedback. This could be any or all of the following:
Your name, address, age, date of birth, skin type.
Occupation, how you heard about us, emergency contact details
Your email address, mobile number, landline number or work number
Your skin concerns
Medical information including and not exhaustive; pregnancy, medical history, current medication or previous treatments you have received and more recently information specifically related to the risk assessments taken regarding Covid 19.

Photographs of the area you are having treated
Information contained in or relating to any communication that you send to us (including the communication content and metadata associated with the communication); and any other personal information that you choose to send to us
If you are purchasing a medical product we will collect the following data to ensure the safe, anti-fraudulent processing of your purchase – Your name, delivery address and postcode
Your email address, mobile number, landline number or work number
Your billing details, including your credit/debit card payment details and billing address
Special categories of personal data are NOT collected by us (this includes details about your race or ethnicity, religious or philosophical beliefs, sex life, sexual orientation, political opinions, trade union memberships, information about your health and genetic and biometric data), EXCEPT for health data which is ONLY relevant to the medical treatment which you may have or are receiving. We take extra steps to ensure this is adequately protected.
We do NOT hold or store your credit /debit card details.

We need to so that we can provide you with medical products and treatments and to manage your account with us.
We need to use some of your information to comply with legal and regulatory obligations (such as medical legal obligations to keep details of treatments had by patients for a certain period of time).

If you have provided consent:
You will receive appointment confirmations and reminders via text and/or email for when you have an appointment booked

We MAY in future send emails or texts about some health news, or treatment advice or other information which we think you may find interesting using the contact details which you have provided: Only if you have given prior permission/consent for this.

From time to time, we may also use your information to contact you for patient feedback surveys. We may contact you by email, phone ONLY IF YOU HAVE GIVEN PRIOR PERMISSION/CONSENT FOR THIS. Please contact Skin Science and Health Ltd directly if you would like to be removed from any appointment information communications from us.

Should you wish to make a complaint about anything, please contact Skin Science and Health directly.

The photographs which the nurse stores of your treatment progress allows us to ensure any incidents are recorded, and we may request further photographic evidence following a complaint to assess the area

We will use contact details to respond to any complaint communication that you send
We use the treatment information to be able to assess exactly what you have had prescribed.
We will NEVER without your express consent, supply your personal information to any third party for the purpose of their or any other third party’s direct marketing. If you choose not to provide personal data:
In order to provide a medical service/ treatment we will need to request your personal information by law, and if you do not provide that data we will be unable to fulfil our duty, so we would be unable to provide that medical service. Further information regarding this can be explained with you at the time.

How long is your information stored for?

Your personal information including treatment information, images and buying data is required to be stored for certain periods of time due to compliance with the law and insurance purposes.
Client records are kept including medical data, images and treatment data in accordance with Records Management NHS code of Practice as reference of good practice.
In order to comply with legislation regarding the retention of financial data, all companies must hold their financial data, including details of whom goods have been sold to and purchased from, for a period of 6 years from the end of their last financial year. As a company, therefore records are retained for 7 years to ensure complliance with the legislation in place

Security:

Access to your personal data is limited to the medical professional, or medical secretaries/ office administrator, who only have a business reason to act on Skin Science and Health’s instructions and are subject to a duty of confidentiality

Your Rights:

As a data subject, you have a number of rights. You can:
Access and obtain a copy of your data on request;
We want to make sure that your personal information is accurate and up to date. You may ask us to correct or remove information that you think is inaccurate and we would ask you that you keep us informed of any changes during your relationship with us.

When a request for a copy of your information has been received, you will be provided with a copy of your records within a month.
